If it had lasted another season, I'm thinking that Firefly would have revealed why Companions have high status. You would not need to suspend disbelief forever because Joss Whedon would have eventually shown the reasons. While we were waiting for the big reveal, we just assumed that somebody classy like Morena Baccarin would not go into a career that deserves no respect, like my career at Burger King. (I love BK Whoppers like Inara loves fornication. That's why she & me work where we work.)
Later edit: I'm trying to think like a science fiction writer. Conditions haven't changed that much from the days when Moses brought the ten commandments down from Mt. Sinai, as least as far as sex. What do we have that Moses didn't? Penicillin, reliable paternity tests using DNA, and birth control with side effects. Now imagine a distant future that has many more technological advances that are related to sex. And imagine that Companions created these advances, either directly as researchers or by funding others's research. I think it follows that there would be changes in what is considered sexually moral compared to the days of Moses. I think Companions would be the vanguard promoting these moral changes. Companions might be considered praiseworthy for what they accomplished over centuries. That's my theory about why Companions are held in great esteem.
By the way, we ain't livin' in the 26th century, so don't get too advanced in your sex life. You might be getting ahead of science. There are practical reasons for Abrahamic religious traditions governing sex. Reasons that go beyond 'God Commands You!' and 'The Bible Says!'

I pretty much agree, Two, and I'd also add that as a regulated incorporated guild under Alliance law, the Companion Guild has to meet all applicable futuristic discrimination laws re: ageism, lookism, classism, and weightism.
The Companion Guild probably has an extensive scholarship program they award girls of all spectrums of society and appearance, and anyone who completes the required curriculum becomes a certified companion. I don't imagine that curriculum being easy, though, requiring intelligence, education, sensibility, and eventually proof of skill.
