And this can't have anything to do with, say, their personal issues? Mal calling Inara a whore is about 25% about her being a Companion and 75% about HER being a Companion, considering the obstacles it places in the path of their relationship. Not to mention, Mal is not the measure of all society, now and in the future. He's one fictional character.
The fact that you can't think that anyone could respect a prostitute is pretty irrelevant. It used to be actors were disrespectable scum in the eyes of society and look how that changed.

In the case of Mal and Inara you are seeing a culture clash (deliberately.) There are cultures that have had the idea of the Courtisan, basically a woman employed to be a companion to a wealthy man. Such women were often highly educated and afforded a high level of respect in those cultures. It was very common in the east China and Japan for sure and maybe other places. It was even relatively common in Europe especially amongst continental royal houses.
Now if you had been an unwashed barbarian Yankie wondering the orient in the 1850's and someone told you that the pretty girl on the local chieftan's arm was paid to keep him company you would probably think "whore" because that's the culture you were raised in. Mal is that Yankie (or given the context of the show that confederate.) Firefly is an eastern-western. You get Geisha and Gunslingers, Manderins and Madam's and the culture clashes make it interesting.

I won't argue any of that except that I really can't believe that I'm hearing from anyone that any of these women are treated with respect.
