I don't think anyone's mentioned it, but I just remembered a little gem from 1989: Moontrap staring Walter "Chekov" Koenig. Terrible. Embarrassing low-G wire fu, space strippers, and Walter Koenig playing a tough guy. Just terrible.
However, my vote would go for a stinker of a series back in the early 80's, called (I believe) "Automan", about a computer-generated holographic crimefighter or some such. I remember that he worked best at night because of the power needs for him to appear. Close to sunrise in the one episode I remember he started fading, talking about feeling the thousands of toasters and coffee makers switching on all over the country. REALLY bad stuff.

What I saw of Cleopatra 2525 was awful. true, but it did have Gina Torres...
Also, i liked what they did with cyberpunk, there. I saw one bit where the ditzt bimbette had to do surgery to remove something, on the fly. There was a holographic overlay to talk her through it, which, i have to say, was five minutes of viewing time not wasted on, say, reality TV...

I think I have everyone beat, but I have not seen all of the movies listed here.
Worst movie I have ever seen, outclassing Alligator II, Armagedon, Deep Impact, Meteor, Asteroid, and MST3K movies in all around suckyness, is Mosquito.
The thing is the same as the movie suggests, giant radioactive mosquitoes. This beat out the other one word titles (Octopus, Creature, Arachnid, Aracniphobia, Python, Boa) night of titles (Night of the living dead, night of the comet) machine taking over ones (maximum overdrive, car) who is the alien ones (starship troopers two, the pod people) post apocalyptic ones, (knight rider 2010) war in heaven ones (prophecy three) and generally all other junk I have watched.
The only movie that I can see coming close lives only as a vague memory and the thing playing on the oracle’s tv in the original matrix. Night of the Lepus. I call it “Attack of the six foot bunnies”.

Sadly for Colin Baker, it was down to the scripts. Any fan who has seen the "Stranger" films he did afterwards (with a certain 'Miss Brown') will know that he's actually VERY good at being a good Dr Who. His role was ruined by the editorial choice of scripts and style.

Worst ever ... Hmmm ... where to start?
Films:-
Highlander II - only this one specifically - III and IV were better, if not exactly GOOD.
Dr Who - The TV Movie - it lost everything which made Dr Who what it was, by turning it into another "pap for the typical American viewer" film. The TV Series was just variable and cheaply done (but see below).
Short Circuit II - sorry, this was just 
TV Series:-
Doctor Who - Colin Baker seasons - these were the exceptions to me liking Dr Who, NOT because of Colin Baker, but because of the way his character was being written. It marked the beginning of the end for the Doc, and nothing which Sylvester McCoy did could save it after than.
ST-TNG - any episode where Wesley Crusher saved the crew To quote Doglas Adams - "the one thing no-one could stand was a smart-ass". And he WAS, in spades.
Space:1999 - second series particularly . Season one was ok in places, and the Eagles looked like someone had really THOUGHT about their design ... but Series 2, targeted for the US market, was a heap of dingo's kidneys.
Dishonourable mention - Flash Gordon (movie version) for just one TERRIBLE joke - "What do you mean, Flash Gordon approaching? Despatch war rocket Ajax to bring back his body."
For those who don't live in the UK, 'Flash' and 'Ajax' were two different brands of floor cleaner.

Lexx, Highlander 2 stand out the most....though I do love the MacLeod movies, the second one stinks..
disagree about the DS:9 posts....that sereies is unlike any other trek series that was made, and it ttruly isn't fair to compare with one of the other best series I have seen: Babylon 5

Ladies and Gentlemen, we have a new contender. A little piece of gos se called Alien vs. Predator. Such a great premise executed so poorly.
Someone mentioned the Ator movie staring Miles O'Keefe. Worse yet was its sequel, The Cave Dwellers. At least Cave Dwellers was done on MST3K, and was hilarious.
Space Mutiny was horrific, but if you watch the MST3K version, it is fantastic. "Big McLargehuge" "Rump Slabfist" "Rock Hardpecs" It's classic.
Anybody remember Space Hunter starring Molly Ringwald or Cherry 2000 with Melanie Griffith? How about A Boy and his Dog with Don Johnson? Terrible!
As for TV, the V series was heinous. The two miniseries were good, but the weekly show they tried to make was disasterous.
