In 'Those Left Behind', while Inara and Simon are trying to calm her, River shouts out "BELLY!"

Then afterwards she says "Not yours. Not Her's."

Zoe's?

However, at the end of Better Days, we find that Inara had Simon in her shuttle, though she inists it was cos he's a doctor.

What did she need a doctor for?

Am I reading way too much into it?
