Why shouldn't Early know about the hatch? Serenity isn't a homemade spaceship, it's a standard model of which thousands were made. Think DC-3. What you're asking is like wondering how a stranger would have been able to walk right up to your car and open the back door. How could he possibly know that Accords have four doors?

Even if Early didn't know the details of the Firefly design, he could easily have called up that information from the cortex as he "hunted" River down.

And of course it's an airlock. You didn't see the air all blow out of the ship when he entered, did you? Unless there's some fancy new technology that lets you open a spaceship door without losing pressure -- which doesn't seem likely given that the main cargo door has an airlock.

I'd also thought about the possibility of the Operative becoming a shepherd, and that Book might once have been an operative. It doesn't appear that he has the Operative's martial arts skill, though perhaps he's either just out of practice or deliberately holding back.

I like to think, though, that Book's past was more complicated than that.

Not that he had NO martial arts skills; they just weren't (or weren't shown to be) at or near the Operative's level.

One would think Book would use these operative skills as a knee jerk reaction. Honed to react to danger with brute force. But if he really spent a great deal of time in some brotherhood, he may have lost his edge. He did seem pretty good at blowing kneecaps though. You notice how many times in the series he walks right out and confronts people without a weapon of his own. He takes control with his presence like Inara uses with her customers. He certainly didn't loose his power of persuasion in the abby.

I'm with most people in believeing some of the words River hears are not what people are thinking at that moment, but some are from thoughts buried in their minds from the past.
Jayne could be a direct thought. He sees River walk in and out jumps his guilt. But Simon is laughing and joking with Kaylee, a don't think his immediate thoughts are about what he lost, although I'm sure he has these thoughts.

I just watched this ep for the first time a few days ago, so thought I'd chime in with my first impressions. I'd have to say that I think River IS reading everyone's thoughts at the moment they're thinking of them. I mean, a recent thought vs. an old memory. When Book's having his "flashback", he was joking about Jayne taking orders and how ludicrous that would be. He might have been thinking back about how un-preacher-like he himself used to be, at the moment River read his mind.

As for the others: Simon was thinking, "I'd be there right now" just as he was reminiscing about the hospital. Jayne's thought refers to his "Saint Jayne" joke, and that he's anything but. Inara telling Mal she's a big girl and to just tell her (his feelings) already. And Mal thinking about his conversation with Inara, and how none of his words say what he really means.
