Some I keep in separate folders...like
Opus
Christmas
Halloween
Couples...for these kind of smilies...
Bananas and other dancing fruit
One for my website
and a whole bunch of loose ones!
I keep a list of what's already uploaded and ready to post...otherwise it only takes me a minute to upload the smilie.

If you open the image in MS Paint (or some other image manipulation program e.g. Photoshop), you can save the image as a .jpg file, then upload to photobucket.
