Heres my idea of a spinoff: Surgeon.
Simon Tam leaves Kaylee River and the rest of the crew on Serenity to satisfy his inner demons. He goes back to the rim planets exacting vigalante style justice and unforgiving, if not unforgiveable, revenge on the people that hurt his sister. He begins systematicly eliminating the doctors, nurses, surgeons and other people involved with the destruction of rivers brain.
You can fix anything if you have a big hammer and enough duct tape.
