The Serenity deaths didn't shock me (well the one was so sudden) because I knew Joss' work well enough by then to expect it.
What shocked me was the the Buffy episode where Angelus kills

SpaceCowgirl: Ditto the Angelus thing. When that happened, I definitely flinched. It wasn't just because it was sudden or unexpected (which it was) - it was the way he did it, and the utter and complete disregard he showed her. For a vamp to just snap someone's neck and not even be interested in drinking their blood means (to me, anyway) that the vamp regards them as garbage, or less. And the things he did with her after that had me really furious. My wife and I both swore we could never like Angel/Angelus again after that. Of course, we were both wrong. :)
