I am sure this has been debated over and over, but I am new to the forum, so please forgive if this has been said already.
My take on the syringe is that (if I remember correctly), they were concerned that they may be getting attacked by Reavers and Inara was going to use that syringe (filled with a poison?) to keep herself from going through the horror of what was to come. Perhaps. Or not.

'Cause as sure as I know anything, I know this: they will try again. ...they'll swing back to the belief that they can make people... better. And I do not hold to that. So no more running. I aim to misbehave." - Mal

That was my thought as well, that it was a suicide device. However, on the commentary, it is expressively said that it is not a suicide device at all.

Wash: Yeah, but psychic? That sounds like science fiction.
Zoe: You live on a spaceship, dear.

One of the writers (I believe it was Tim Minear) spoke at a panel discussion and said that it was a drug that would cause death to anyone who had sex with the person who took it. So, the Reavers would have died if they had raped Inara.

Did he just go crazy and fall asleep?

Thank you both very much for the info. I have yet to watch with the commentary, and had not seen/heard/read the interview you speak of, so I was not aware of these things.
Again thank you both for shedding new light on this for me.

It's not an interview... just summaries by fans who attended the panel:

Select to view spoiler:

I just thought that it was a “THE PILL” thing…you know like the
Cyanide pills that spy’s have in moves. You know you get in a sticky spot
And bam you take the pill and you die then it is over.

I am spoiler this because I do not want to be rude or offensive to anyone.

I also thought on the very first (and only! ;_;) viewing that it was supposed to be a suicide drug... it never really occured to me to think otherwise. They were in mortal danger, facing a fate worse than death, and Inara strikes me as being willing to die peacefully rather than... that way.

I realize that the commentary says otherwise, but how were we supposed to know? Was this going to come up later in the series? I guess we'll never get to find out... stupid Fox...

I have to agree with the masses and admit I also thought it was a suicide device. I wonder what ques where there to say otherwise.

I hear ya on the anger toward Fox. They love to cancel really good shows. Not only Firefly, but they just canceled a show I really liked called Reunion. **shakes head**
