Vader. Hands down. Come now, no matter who you chose to list, Vader came to mind... probably first. Are all of the star wars movies top notch? No. Was Vader the best villian of all time? Heck yes.
As for others, I'm torn, but I think I'll go witht he T-1000 from T-2, the early borg, (when they were just cold hearted and efficient, before they became retarded) and the Shadows from Babylon 5.

1. Aku from Samurai Jack. He plunged the Earth into hundreds of years of darkness, he had those scary flaming eyes, and he so much enjoyed his evilness!
2. Number Two from the Prisoner. Since there was a different Number Two for each episode, I liked that it was the position, not necessarily the person who was in it, that was evil.
3. Lindsey from Angel. Especially with the Evil Hand.
