The British and other colonial powers took a lesson from the Romans and 'Balkanized' their former colonies, putting different ethnic/ tribal groups together in single countries, setting up minorities to be the ruling class, etc. My understanding is that that's what happened in Rwanda under Germany. And then when Germany attacked Belgium in WWI the Belgians attacked Rwanda in retaliation and took it over from the Germans. And then they REALLY went overboard, setting up the minority Tutsi to be the overseers, owners, and rulers over the majority Hutus under Belgium's auspices.
Since that division was carried forward even after the country gained independence, there was a lot of bad blood between the two groups.
Generally people in power who garner a big share of the wealth won't give it up peacefully, and find ways to rig the system to keep things going the way they are.
There's a lot of sins of the past there, as in many places, like for example SA.
I don't know what other external individual countries should do about those situations. For example, the US didn't cause Rwanda's problem (though it certainly has plenty to answer for elsewhere!). And I'm not sure the US has the responsibility - or even the right - to control other nations and run them to its liking. I can imagine how Americans would feel if China were to take over the US and start re-drawing state lines for example ... just because it was bigger and stronger and disliked how we do things. And a LOT of awful stuff was done and a LOT of people were killed just b/c the US claimed RTP - right to protect. As an example look at Libya now - a smoking ruin of Jihadists with open slave markets - that used to have security, a high standard of living, and higher education for all - yes, even women.
Now, I don't think one can right the wrongs of the past (like Biden* wants to do with reparations). As the song says, the past is gone. But one can work to address the wrongs of the present. But who, or what organization, exactly is going to swoop in on countries to tell them how to fix things so that you don't end up with Rwandas everywhere? Who exactly is going to say that violence needs to stop and that it's not warranted or in service of long-overdue justice?
Anyway, those are my thoughts, but I have no answer.
ETA: Canada stands as a shining example of a modern country that's attempted to set things right in the here and now.

If you look at the map, it's a stunning amount of land to re-designate in modern times. But even though I was fully an adult when it happened, and lived w/in 5 miles of the Canadian border, and actively followed Canadian news via CBC radio, I have no idea about what could be the rationale for that result.
If Canada was to try and reverse entirely the past injustices, it might have said - we took all your land by force and trickery - here, you can have it all back. But then, that wouldn't have been fair to all the people who put all their work into the land to create what is there today.
And you'll notice Canada didn't give away Alberta tar sands. Or BC's waterfront and tourism areas. Or Canada's financial capitol in Ontario (Toronto), or Ontario's product
