Captain or Mal?

Ok, I've been trying to do this myself, but I get distracted and forget to listen.

I've counted three times, I think, when Zoe calls Mal "Mal" instead of "Captain" or "Sir" -- once in War Stories,to Wash, once in Trash,to Inara and I know I heard at least one more, but I can't remember where.

Right now, I only remember Kaylee calling him "Mal" once, and that's when she's introducing him to Simon in Serenity.

Anybody got any others?

And does Book evercall him anything but "Captain"?

Does Jayne ever call him Captain?

I think the doctor only ever calls him Captain.

Jayne does call him Mal, espcially in 'Jaynestown.'

Jayne calls Mal 'Captain' in Serenity but not to his face. He tells Dobson something like "Does helping you mean turning on the Captain?"
