Hey folks! This question has probably been ansawereda bazillion times, but I haven't been on as much as I used to.

What did Simon inject into Mal's arm right before the heist in the BDM? I assume adrenaline.

In the hallway where Mal first runs into Simon, Mal says “Better get innocked before we hit planet-side.” The line is hard to make out (I always hear it as “Better get her knocked before we hit planet-side”), but it is meant to explain what the injection was about. Mal is getting inoculated (“innocked”) against the local diseases on the planet so he doesn’t pick up a bug during the heist.

That said, the real reason for the injection of course was to reinforce the notion that Simon is a doctor for those watching the film for the first time (“Look, see: he’s doing doctorly stuff!”).

Everything I've seen is speculation. It ranges from drugs to an inoculation.

So, unless someone has found something from Joss, in the Visul Companion, or somewhere else, we're left to the guessing.

EDIT: Ops, explained. I guess I should hit refresh before posting when I open lost of windows.
