I've read lots of posts about OOG so I thought I would put my ideas in one thread instead of in lots of answers.
first i think FF is the best Sci Fi ever and OOG is the best episode in so many ways.
The end scene with Mals face as he sees Serenity for the first time. WOW it seems to me he is drawn to her cos she's abandined derelict an broke just like him. so all the stuff in the episode showing him gettin her ready an crewed its all about mal finding some purpose in life hence in the pilot he says to Simon
"we're still flyin"
Simon "That aint much."
Mal "Its enough"
so when she breaks he can't leave her cos if he does then shem is abandoned derelict and broke and so is he! he doesn't stay to die he stays to fix her somehow.
Also though he says to Inara "Everyone dies alone" it seems like the ultimate cinical comment but he aint no cynic he's a romantic its just a protection for him, so when he wakes up and sees the crew round him and says "did I call You back?" and they all say "No" that big smile is cos he knows that whatever happens he won't die alone they will always come back for him.
Finally someone said how come one man with a little gun sees off a whole bunch of fellas with big guns I think the exchange between Mal & the scavanger says it all,
scavanger "You'd have done the same."
Mal "we've already seen I didn't."
Mal has & the BDH's have a form of moral courage that means they would die for each other (including serenity) the scavangers don't & so none of them will risk being the one to get shot.
Anyway sorry its so long just wanted to get it off my chest. They're just my ideas love to see your thoughts on it though.
Keep Flyin

He's lookin to shoot some folk
