actually, i think that it looks like he has been forced into suboptimal emergency care conditions, and is giving a dirty look to someone (probably wash or mal) for making a stupid joke.

but that's just my take on it.

Very stylish. That's a great concept and pencil sketch. Are you planning on pursuing this? Or maybe since we have the addresses of the stars, I hear they love to receive original fan art!
